What is it?

 

 

P&C Staff agree to:

 

The community agrees to:

 

  • Conduct a survey to identify priority needs, identify leaders who need to be, and who are willing to be, involved in developing solutions to the needs, and identify possible alternative solutions

  • Identify participants for Key Informant Survey and facilitate survey meetings

  • Facilitate the work of the community partnership board to select which needs the members and participants wish to pursue

  • Review P&C recommendations on alternative structure and leadership and select measures that strengthen the board

  • Assist the community partnership board in establishing an organizational structure

  • Recruit new coalition participants as needed

  • Link the community with other partners that will provide specialized technical assistance, financial assistance, or assist in identifying needs and priority areas

  • Work with the Commission to secure staff support where needed for the board and its committees

  • Support the partnership board in efforts to implement and monitor the agreed action plan

  •  Provide leadership for follow up implementation of agreed upon program and policy objectives in the community

  •  Assist with the development of evaluation strategies

  •  Enhance board organizational structure to sustain agreed plan and project monitoring function

  • Provide financial support in helping the community meet their identified goals
